    • Hi soledad, yes, you can get your husband’s MDR from Philhealth by presenting your marriage certificate (orig and xerox) and an authorization letter from him plus a copy of his IDs. Ask your husband to email you his letter and you will print it.
If the Trece branch is near you, you can go there and request for your husband’s MDR using only your marriage certificate plus any ID he has left with you or a copy of his OEC or seaman book (baka naman puede without authorization letter)

    • Hi maryann, you need 450 pesos to pay for the initial 3 months or one quarter. Are you the one enrolling? Or is it your papa? If it’s you, bring your birth/baptismal certificate or valid ID. If it’s your papa, bring his ID or birth cert and his authorization letter. Bring your ID.

I was previously employed from february up to april. And i want to continue my contribution. My concern is this. I am pregnant now my due is on october and this is bugging me, will i be qualify to use my philhealth by the time that i will give birth?

    • Hi khrisel, the requirement for voluntary members (maternity case) is payment of 9 of the 12 months BEFORE month of delivery. In your case, parang kukulangin ng 1 month, kasi Feb to Sept is only 8 months. Please ask others too.

  10. Hi Sir/Ma’am,

Is there a minimum or maximum monthly payments of contribution for different kinds of payee like, business men, ordinary employee, self employed or voluntary payee, etc. ????
I wasn’t given yet a membership card when I applied due to unavailability of it on that time, can I request for it at any branch nearest to my place now? ( I applied in Cabanatuan city, now I live here in Carmona, Cavite).
What are the exact benefits from Philhealth that any member could get? !And for how long?
Lastly, is the contributions refundable when never been used or consumed throughout the members life?
Thank you and more power to the company!

    • Hi teresita, for most individual payors, the rate is 150 pesos per month. For individual payors earning more than 25k per month, the rate is 300 pesos per month. About IDs, as of my last inquiry, wala pa raw available at any branch. When available, yes, you can apply for your ID at any branch.
For voluntary/self-employed members, they should be updated in payments for at least 9 months BEFORE month of hospitalization (for maternity and surgeries). 3 monthly payments are enough for ordinary hospitalization.
About refund of premiums: No, premiums are not refunded. They’re counted so that you can be eligible for free lifetime membership when you turn 60 (you need 120 contributions).

    • Hi vilma, yes, if you get a Philheath no. at the branch, it means you’re Voluntary, and they will ask you to pay the first quarter, which costs 450. Employers naman kasi dapat ang magre-register ng employees nila. Tell your nephew to get his Philhealth no. sa philhealth.gov.ph, click Online Services, and then Electronic Registration.

If younger than 60, you need to continue paying if you want to be covered. If you decide so, you can go to Philhealth with your ID, renew your registration and pay 450 (for 3 months), and then later on pay 150 (Bayad center, LBC) for every month thereafter.

    • Hi Nhel, ask your family to ask the bank that issued the check (Landbank?) if they can encash it using a SPA from you because you’re in Saudi and that you don’t have a bank account. The other option will take a long time — write Philhealth with copies of your IDs to reissue the check in your wife’s name or mother’s name or sibling’s name.
What do you mean ipinagbawal na? Kahit yong sa BDO kabayan account which can be opened at BDO remittance partner companies?

Starting Oct 1, nine previous monthly payments na ang required. So kung Apr to Sep lang ang paid by Oct 1, hindi mo pa magamit on Oct 1. If by Dec 1, paid na ang Apr to Dec, puede na gamitin in Dec.

Get Philhealth membership form (puedeng download from Philhealth website) at papirmahan mo sa father, and then get his ID or birth cert plus marriage cert (para maregister na dependent ang mother) and then submit the registration form at Philhealth. Bring your ID. Bring na rin authorization letter from your father-in-law para lang sure. 600 pesos per quarter. Puedeng 1 quarter muna, or puedeng 4 quarters or 1 year.

If not, iregister mo lang yong isa, maybe your father, as member at dependent ang iyong mother, para isa lang ang babayaran mo. 200 pesos per month. Minimum of 600 pesos (3 months) sa initial payment. Get a registration form (get from Philhealth or download and print from philhealth.gov.ph), accomplish it, have it signed by your father, then bring the form and his ID or birth cert plus authorization letter (para sure, although minsan hindi na hinahanap). Bring also your parents’ marriage cert to support your writing of your mother as dependent.

    • Hi Ailene, yes, that’s true that you need to be updated with your payments to be able to use Philhealth. If you’re on long vacation, pay Philhealth as Individual Payor. This month, pay July to Sep. You can no longer pay for past quarters. For Individual Payors and for seafarers, you need to have paid 3 of the 6 months prior to hospitalization to be eligible for Philhealth.
Your premium payments for past years (at least 120 monthly premiums) can be used ONLY for Free Lifetime membership application when you turn 60.

    • Hi Princess, maybe the 3k was only for downpayment and the bill was really 6500. The 8k is further allocated as 6,500 for maternity and 1,500 for prenatal care, if prenatal ORs are submitted. If the total bill is lower than 6500, the excess will be given to the lying-in and not to the patient.
Submit prenatal care worth at least 1,500 so you can get a refund check of 1500 from Philhealth.

    • Hi Edjie, are you sure you transacted with a legitimate Philhealth officer? What I know is the Philhealth ID is free. If you were made to pay for 1-month premium or 3-month premium, it should have been 200 pesos or 600 pesos, and not 300 pesos. What item or transaction is written on the OR?

    • Hi veronica, is your mother 60 years old or older? she can only be your dependent or your brother’s dependent if she’s 60 or older. If 60 or older, ask your brother to go asap to Philhealth with his ID, his birth certificate (xerox and orig) and your mother’s senior citizen id or birth cert or any proof she’s 60 or older and update his MDR so that your mother is written on his MDR.

  69. Hello! I have unified id.I’ve worked in Pinas, then resigned. I have certificate of Philhealth Premium Payments with O.R nos. but I have no philhealth no. I worked abroad. Based on the OWWA Insurance program, I paid philhealth but don’t know the coverage. I decided to work locally, and they require my philhealth no. Is that different from UMID? How do we get Philhealth no? thanks

    Reply
    • Hi Tin-tin, as of now, it’s SSS that’s actively using the UMID no. To get your Philhealth no., go to Philhealth and fill up member data form. Check FOR UPDATING. Philhealth will give you your Philhealth no., MDR and Philhealth card. If you’re sure you’ll be hired, then don’t pay for Oct to Dec 2014, but if not yet, you can pay Oct to Dec 2014 (600 pesos) so you have coverage for some months.

    • Hi Karla, hindi na puedeng bayaran ang past quarters or past years. Merong payment deadlines. Ang bayaran mo ngayon is Jan 2020 to March 2020 (1st quarter) para mabayaran mo rin ang January. Then puedeng monthly, or every 6 months or every year. Kung halimbawa kelangan mo nang gamitin, babayaran mo lang yong mga months na dapat nabayaran mo in the past plus interest.
