To register for Philhealth membership:

– Bring your ID or birth certificate

– Bring a copy of your marriage certificate and/or children’s birth certificates to register your spouse and children as your dependents

– Bring money to pay your Philhealth premium for at least one quarter

    • Hi jocelyn, it may take time for Philhealth to contact your employer. Please follow up with Philhealth. Or you might like to also file your complaint with DOLE.

    • Hi roseann, nag-issue na ang Philhealth ordering hospitals to accept MDR and premium payment receipt as proof of eligibility kasi wala na uling available na mga Philhealth IDs. You try asking at SM Business Centers if they’re still issuing Philhealth IDs — although if they are, merong bayad kasi doubling as discount card din.

Good evening po..diba to register kelangan ng valid id at birth certificate issued by NSO and libre naman pagawa ng id..ask ko lang po now kung meron pa bang mga id silang binibigay now.balak ko po kasi mag register tomorrow baka kasi wala ng id na dini-distribute or naubusan sila..tsaka po magkano naman po yung philhealth premium for atleast one quarter po?Thank you in advance po..

    • Hi Carlo, sorry kung hindi na aabot itong sagot ko dahil nakapunta ka na sa Philhealth. I think wala pang mga available IDs, kasi ito ang nababasa ko online. Pero it’s great kung meron na. For voluntary members, the premium is 150 per month, or 450 pesos per quarter. Yes, bring your ID and birth certificate.

I would like to ask if the ids are already available for distribution or nothing remained for distribution because I would like to register and apply for the id tomorrow in Calamba. Also do I need to make a letter of request for the id and how much is the required payment for premium at least one quarter, is it also required premium payment for applying an id or an optional?     • Hi Carlo, you don’t need to write a letter of request to get a Philhealth ID. What I read online is that Philhealth IDs are not yet available. Voluntary premium rate is 450 pesos per quarter.

    • Hi ivie, yes, yan pa rin ang gamitin mo para yong past payments mo mabilang for your free lifetime membership later on. Sa Philhealth ka muna magbayad para makakuha ka ng bagong MDR at ID kung available na. Fill-up the registration form to update your data. Bring your IDs.

I’m quite confused with premium requirements ng Individual Paying member. ang nakalagay sa site is 300php can be paid quarterly or 1,200 per year for those IPM’s earning 25,000 and below/month. Pero sinabi po sa amin sa ospital na pwedeng kahit 100php/quarter lang ang contribution. so, magkano po talaga? Thank you!:))

    • Hi Kaye, nag-increase na. At buti hindi natuloy na from 100 to 200 kundi to 150. For many individual payors, the rate is 150 pesos per month. For those earning more than 25k per month, the rate is 300 pesos per month. Merong mga programs ng mga ibang barangays or associations or DSWD offices helping families pay their Philhealth so merong cases na mas mura ang per month.

  9. hi ma’am good afternoon! i’ve signed the membership form online just today but i’ve been a member since before.. i am an ofw.. my question is can i submit those requirements through my representative like my sister?.. and what are those requirements that will be needed?.. because i’ve just add my daughter as my dependent.. can i pass it at any branch preferably in calamba?..
    my other question is can i pay my contribution through my representative too?.. thank you and more power!

    • Hi jhossa, yes, your sister can file papers for you, at any Philhealth branch. Give her your Philhealth no., OEC (orig and xerox), or other proof of your being an OFW like work visa, etc. and your daughter’s birth certificate (orig and xerox). Pati na rin authorization letter, your sister’s ID, baka lang hanapin. Yes, you can pay through your rep — 1800 for 1 year, or 450 pesos for 1 quarter. Tell your sister to inform Philhealth that you have enrolled online.

  10. Good morning po,self employed po ako. Pwede ko po ba malaman kung magkano po ang huhulog ko per month?
    And last June 6 2013 lng ako ako ng member, kailan ko po kaya makukuha yung card ko and pwede po din ba magpabID dun?

    • Hi amelyn, it’s 150 per month. If you pay at Philhealth, minimum payment is 1 quarter = 450 pesos. You can pay at Bayad Center if you like monthly. Bring your Philhealt no. About ID, sorry wala pa raw, sabi ng mga nagco-comment dito.

    • Hi Gina, okay lang ang merong gap, basta huwag lang more than 3 months ang gap. Or kung sa maternity or surgery, nabayaran mo ang 9 of the 12 months BEFORE confinement.

    • Hi Joan, kung ngayon ka lang ma-late, baka puede pang bayaran ang April to June, kasi merong grace period for good payors. Dapat sa Philhealth branch ka na magbayad. Bring past receipts to prove you’re a good payor.

      • Hi lorina, there’s no penalty, but pay asap. Some will ask how were you able to get your OEC without paying your Philhealth. But go ahead and pay. Pay through IRemit or Ventaja, or ask someone here to pay for you. They should show proof of your being an OFW.

    I would like to ask, can I still avail philhealth deductions to
    My hospital bill just in case I’ll undergo inpatient general
    Check-ups this month even though my last premium payment for Updating was last march of 2013.
    do I still have to pay the months of April to present for me

    • Hi glenn, sorry ang alam ko hindi covered ang inpatient general checkup. Kelangang merong illness at treatment na isusulat ang doctor. Ask others too.

    • Hi MaryGrace, walang bayad ang mag-correct ng name. Bring your birth certificate (xerox and orig) and ID. Fill up registration form. Check FOR UPDATING.

Hi! yung inaanak ko po kasi na balian ng buto sa braso, kaso yung parents po niya hindi po active member ng philhealth. pwede po kaya na magapply na sila ngyon ng philhealth at bayaran ang contributions na namiss out nila para po makaavail ng philhealth benefit pag naoperahan na yung bata next week?

  16. Hello, i am an OFW and i filed my membership august last year. i paid for one year. Now i would like to pay for this year but when my father went to your office in san pablo, they asked for so many documents. Ano po ba ang kailangan for easy payment? Thank you very much.

    • Hi joyce, email your father a copy of any proof that you’re OFW, such as OEC, working visa, or Foreign ID, or overseas work contract. Include a scan of your ID. Pati na rin authorization letter authorizing your father to pay your premium. Your father brings his ID, para sure na. Mas flexible kasi ang eligibility ng OFW compared sa Individual Payor, so Philhealth needs to be sure na OFW ka.
      By the way, this blog is not administered by Philhealth.

    • Hi Ericson, sori I don’t have this kind of list, so mag-ikot na lang kayo in person and look for the Philhealth sign sa doors ng dentists, kasi parang bihira ang accredited dentists. Additionally, hindi ko sure kung covered ito dahil considered tooth extraction lang siya although much more expensive than the usual tooth extraction. So magtanong na lang in person.

    • Hi armie, wala pang online system for MDR issuance. As of now, SSS no. and Philhealth no. are different nos. Pero kung meron ka nang UMID SSS ID, puede na rin itong gamitin as Philhealth ID. Later on, magiging 1 number na for at least 4 govt agencies, pero hindi pa sa ngayon. Sa hospital, ang hinahanap ay MDR at receipt or cert of premium payments.

  19. good day! this is rhemon ask ko lng po if we can avail reimbursement sa philhealth. hindi kc accredited ung maternity clinic na pinanganakan ng wife ko.i am active member since 2005 til present but may namiss ako payment jan2013 to feb2013.i work from 2005 to 2012 sa isang manufacturing company.by jan2013-feb2013 un lng wala aqkong contribution.can we avail reimbursement?and what requirements para makaavail?pede ko p bng hulugan ung jan and feb2013 khit may penalty?

    • Hi rhemon, sorry hindi kayo maka-avail kasi hindi Philhealth-accredited ang clinic. Also, dapat nabayaran ang 9 of the 12 months before month of delivery. Pero starting Oct 2013, puede na ang 3-month payment for the Informal Sector.

    • Hi ericka, ask the father to update his Philhealth record, showing birth certificate of baby and his ID. He can do this through his HR dept

    • Hi elaine, if you filed your Philhealth papers with the hospital or with Philhealth, wait for your benefit payment notice via registered mail. But if you have not filed, sori, late na po. Claim filing should be within 60 days after discharge.

  23. hi, manganganak ako nxt yr. employed ako til feb-july this yr. bayad nmn lahat ng month na un, ang tanong ko lang, pwede ko ba bayaran as voluntary member un contribution ko ng august-december? tas pwede narn kaya magadvance payment ng jan-march2014? at anu ano kylngan kong dalin sa panganganak ko? cs ako, totoo bang 19k ang mababawas sa bill ko? thanks!

    • Hi claire, pay Oct to Dec asap, (you’re late for Aug and Sep) and then all the 2014 months before your delivery. Request for a cert of premium payment from your former employer. CS should be Indicated CS, meaning decided by your OB. 11400 is for hospital; 7600 is for doctor. Depende rin sa OB kung ayaw niyang irefund o ideduct yong 7600.

    • Hi stef, individual payor ka ba? If yes, go to Philhealth and change your category to Informal Sector. Ask if you need to pay 1 year in advance under Informal Sector Category so you can avail.

  26. Ask ko lang po ilang months dapat maayos yung philhealth re imbursement. I was admitted dec 14,2013 and discharged dec 16,2013. Ano-ano ang needed requirements kung hindi ko kasama yung husband ko sa filing, beneficiary po niya ako. Ilang months po ang waiting time para maayos at mareimburse ang pera. Thanks!

    • Hi Lyns, sana nag-submit kayo ng docs sa hospital, kasi mas preferred na ngayon ng Philhealth ang hospital filing. Processing may take 2 to 3 months or more. Docs: premium receipt, MDR where you’re declared as dependent, claim forms from hospital, bill payment OR, statement of account, waiver from hospital and doctor.

    • Hi April, baka hindi pa siya naregister ng employer at hindi pa naremit ang mga payments niya. Ask your sister if she filled up a Philhealth registration form. Puede ring tanungin niya ang HR nila kung ano ang Philhealth no. niya.

    • Hi Rafhaela, sori, wala pang online system ang Philhealth for change of data. Anyway, just be ready with your birth cert (orig and xerox) and ID so that you can change your data when you have the time to go to Philhealth.

  32. ask ko po about dun sa refund ng philhealth po kung gaanu katagal yung babalik na pera ako po ay indibidual member. ang asawa ko po ang beneficiary ko at inoperhan pa po sya nung last year may sa general cailles hospital, how come po kung montlhy po eh me binabayaran ako sa phlhealth ano po bang makukuha na benefit at wla pa hong nadating na refund. kung one year po ang refund eh di po ba napakatagal nman po nun? salamat po

    • Hi rosario, may natanggap na ho ba kayong benefit payment notice from Philhealth via postal mail? Nag-submit ho ba kayo noon ng Philhealth docs sa hospital at nag-file ang hospital sa Philhealth? Kung 1 year na, mag-follow-up po kayo sa nearest Philhealth.

    • Hi amirah, yes, because you said you paid for the whole year of 2014. It means that your due date of July 2014 is within the validity dates in your premium receipt.

    5 years na po akong employed and I always see Philhealth deductions on my payslips. But I never had a chance to take advantage of the benefit since never pa ko naconfine. Pero ngayon, I’m 5 months pregnant na po and would need assistance how I can check my Philhealth status. Thanks!

    • Hi Jessa, ask for your MDR at the nearest Philhealth branch so you can verify if your employer is remitting employees’ premiums.

    • Hi Jeanette, a father can include his wife and children as dependents. Present marriage and birth certificates. 600 pesos per quarter. 2,400 pesos a year. Brothers, sisters, aunts and uncles cannot be dependents.

Hi, manganganak po ang wife ko sa 1st baby nmin by November. if ever po sa lying-in clinic lang sya manganak at hindi accredited ng philhealth yung clinic. nakafile nman sya as beneficiary ko, maaari ko pa rin ba ifile ang gastos ko sa philhealth.? anu po ang kaylangan na document.     • Hi Francis, you must find a Philhealth-accredited lying-in clinic. Direct filing by a member with Philhealth is no longer allowed. You cannot use Philhealth if the lying-in is not Philhealth-accredited.

    • Hi Lolita, go to the nearest Philhealth with your ID plus 600 pesos to pay Oct to Dec 2014. Fill up member registration form. Check “For Updating.” Write your Philhealth no. if you know; tell officer if you forgot. You’ll get your MDR and your Philhealth card.

    • Hi jenneth, yes, your child can use your philhealth. you paid for 1 year so i assume the date of confinement is within your premiums’ validity dates of Oct 17 2014 to Oct 17 2015. Requirements: email your authorization letter to process your Philhealth as you are abroad and a scan of your ID, receipt of premium payment. Your representative should also bring his/her own ID. Your spouse or child can sign your claim form 1 (they bring birth cert or marriage cert. to be sure). If your child is not yet written as your dependent in your MDR, authorize also someone to update your MDR because there are hospitals which require updated MDR (they bring birth cert, xerox and orig).

    • Hi Bhogs, you can file at Philhealth near SM Southmall, just past Southmall, to your right. But direct filing is no longer allowed, unless you have a valid reason. Your benefit should have been deducted from your bill

    • Hi roselyn, did you submit Philhealth papers to your health center? Is your health center accredited by Philhealth? If your total payment is less than 6,500, then your total payment should be fully refunded to you.

    • Hi maria rizza, yes, you can pay at Bayad Center. If her MDR is not clear, pay at Philhealth so you can ask for a new MDR. Bring her ID and authorization letter

  42. HI, I checked my contributions online and saw june 2014 – dec 2014 payments only. until now I’m employed po. so 1 year and 6 months po that we didn’t have philhealth payments. But I know our employer pays by cheque. then i found out that our employer has not updated our payments. If they will update, will they be posted immediately?

    • Hi jean, yes, your employer should update your payments asap so all employees will have continuous coverage. They will pay penalties for late payments.

    ask ko lang po kung paano ako makakuha ng prenatal/maternity benefits sa philhealth company. member po ako since 2011 hanggang 2016 at kada buwan ako my deduction sa salary ko for philhealth. sa loob ng 5 years kong pagtratrabaho hindi po continuous ang paghuhulog ko kasi magkakaiba company ko kaya may buwan na wala ako hulog. dito po ako nakatira sa san pablo laguna. hindi pa ako kasal sa asawa ko.may maitutulong po ba kayo sa concern ko? first time ko po mag ask ng help sa company nyo kung pano ko magagamit mga deductions ko. here is my permanent philhealth number 04-050-139202-4 thank you.

    • Hi maricel, sorry this is not Philhealth’s website. I just write about Philhealth. When will you give birth? If you give birth in July or Aug or Sep, you should pay for April to June ASAP (payment deadline is June 30). About prenatal care, submit 1,500 worth of prenatal ORs to your clinic/hospital together with your MDR and claim form 1 so that Philhealth can refund you 1,500 later on. If you’ll give birth at a lying-in, you need to have prenatal check-ups there so you can avail of prenatal benefit.

  44. I have a PhilHeath Identification Number na po, but wala pang ID, and I want to print a copy of my Members Data Record, pero pag piniprint ko na, ang lumalabas is “Please Supply Member ID”. What can I do po? Need ko po kase para sa work. Thank you.

    • Hi Irish, okay naman sa akin dito. Baka lang merong glitch sa Philhealth website when you tried to print. Ang gawin mo na lang is i-download mo yong MDR mo, then saka mo i-print.
